Victorian skilled visa nomination applications require additional forms to be submitted. You can download all additional forms below.
It’s a good idea to print, complete, sign, scan and save each form for your records and to ensure each form is ready to upload at the end of your online application.
Online applications cannot be accepted until the relevant documents are submitted.
Declaration form
For all skilled nomination applications a Skilled Declaration form must be signed stating that the applicant (and migration agent – if one is acting on behalf of the applicant) understands the conditions of Victorian visa nomination.

Confirmation of Employment statement
Applicants who have an offer of employment, or are currently employed by a Victorian employer (as indicated in Question 2.1 of the skilled online application form) should complete a Confirmation of Employment Statement.
